Eligibility
Adults 18 years old or older diagnosed with a terminal illness and a life expectancy of 12 months or less, or diagnosed with a progressive disease that will make participating in the Dream impossible within a year. Diagnosis and Prognosis provided by a doctor with the application. Available to people in the United States, including Hawaii, Alaska, and Puerto Rico.
