Application process

Complete and submit application. Qualified senior must provide ID to pick up their box at designated site each month and must recertify at 6 months.

Description

(CSFP) The CSFP works to improve the health of low income elderly persons by supplementing their diets with nutritious USDA foods. Participants must complete an application, be at least 60 years of age, reside on Oahu and have an income at or below 150% of Federal Poverty Income Guidelines. Once qualified, senior will receive a monthly food box consisting of UHT milk, juice, cereal, rice or pasta, peanut butter or beans, cheese, canned meat/poultry or fish and canned fruits and vegetables.
