Eligibility
Adults (18 years or older) who are homeless or at risk of homelessness. Must have the completed Coordinated Entry System (CES) paperwork and case manager.

Application process
Referral Required Case managers from street outreach teams and the coordinated entry system reach out to Kumuhonua only.
