Provides information on Advance Care Planning to help people who may be facing serious illness and their loved ones prepare for decisions they may need to make as a person's illness progresses. Provides information on advance directives and is Hawaii's clearing house for POLST (Providers Orders for Life Sustaining Treatment)a portable medical order for people with advance illness. Provides professional and public education, advocacy and public policy, referrals and resources on palliative and hospice care, and leadership for statewide palliative and end-of-life care initiatives. The "Let's Talk Story" Program will go to community groups, churches and temples and provide free training.

Kokua Mau helps people who may be facing serious illness and their loved ones to understand decisions they may need to make and to find information and resources in our community. Please visit our website at www.kokuamau.org for information on advance directives, POLST, hospice, palliative care and other topics related to serious illness. We are interested in helping people, especially caregivers, prevent crisis and relieve stress.
