Transitional Family Care for youth 5 - 17 years of age with emotional and/or behavioral disturbances, Hale Kipa's Therapeutic Foster Care Program provides community-based services within a family home setting. The 3-9 month program is often a "step-down" from hospitalization or a residential setting and can be helpful in transitioning youth from a higher level of care to successful integration into the community.

The federal Supplemental Nutrition Assistance Program (SNAP) provides crucial food and nutritional support to qualifying low-income and needy households, and those making the transition from welfare to self-sufficiency. The SNAP processing center provides public assistance, food stamps (SNAP) and financial assistance, to eligible individuals and families. They handle new applications and ongoing assistance. A paper copy of the application can be downloaded, printed, signed and submitted via US postal mail or dropped off in the drop box. HomeAid's Kauhale Management Division provides secure, shared housing at Kipuola Kauahale, light case management, job readiness support, and opportunities for education and community engagement primarily for single individuals.  Additional support for mental health resources and counseling referrals, transportation assistance for work or school, and health and wellness programs for residents may be available. Residents have their own "sleeping pod" but share communal space (toilet, shower, kitchen, computer lab, public areas, etc.).  Residents are required to attend weekly or monthly community meetings, help with shared chores to maintain the common areas, follow site policies for safety, treat others with kindness and respect, and stay engaged with job training, education, or personal development activities.

Provides care coordination to target populations. Participates with communities to problem-solve health issues. Provides education in women and child health, chronic diseases, etc. Facilitates access to health care. Conducts screening tests and source/contact investigations on tuberculosis (TB) cases. (Women Infants and Children Nutrition Program) The Special Supplemental Nutrition Program for Women, Infants and Children (WIC), is a federally funded program which provides Hawaii residents with nourishing supplemental foods, nutrition education, breastfeeding promotion and health and social service referrals. The participants of WIC are either pregnant, breastfeeding, or postpartum women, and infants and children under age five who meet income guidelines and have a medical or nutritional risk. Nutrition and Health Education: WIC will answer your questions about what to eat for a healthy pregnancy and help you plan family meals and snacks. Education and support for moms who breastfeed: WIC has specially trained staff to answer your breastfeeding questions. Ask for more information about our breast pump loan program. (MDA Summer Camp) Each summer, kids and young adults with neuromuscular disease attend overnight camps around the United States at no cost to their families. Note: Currently there are no in-person camps in Hawaii. Please see the web site for virtual camp options. Campers may also apply to attend a camp in another location of the nation.

(Sultan Early Intervention Program, Easter Seals Sultan Early Intervention Program) Easterseals Hawaii provides individualized early intervention services to children under 3 with or at risk for developmental delays and their families. Services include: Development of an Individualized Family Support Plan (IFSP); Parent coaching and family support; Developmental evaluations; Resources, information, and referral services; Assistance to families in transition planning for their child’s transition into the Department of Education special education, Head Start or preschool.
