(Transitional Family Homes) Provides a temporary out-of-home placement living environment for youths, ages 12 years old to 22 years old, that require additional resources to accommodate their emotional, behavioral, and mental health issues. Together, the staff and foster families provide youths with a system of support, counseling and skill building services. This service offers youth the opportunity to live with a trained transitional family who provides them with support and guidance. Services are offered to each youth based on their individual needs. Youths are provided with individual counseling and family counseling with the ultimate goal of reunification with their family.

The Caregiver Foundation provides support for the caregivers and individuals needing care through practical and emotional support services and educational programs. Provides comprehensive care coordination, financial management, guardianship, conservatorship, trusteeship, community education programs and caregiver support groups.

Quality Care for Hawaiian Keiki (QCHK) is a child care subsidy program for Native Hawaiian in Maui County. QCHK goals are to help Native Hawaiian/Part Hawaiian families become more economically self sufficient as well as helping their children to become school ready. Program provides assistance to families to pay for child care, for children ages 0-13 and up to 19 years old for children with special needs. Child care includes Licensed Family Child Care Homes, Infant and Toddler Centers, Preschools, Before/After school Programs, Intercession care as well as License-Exempt providers. Parent Participation Preschool is a three-phased program for parents to assist in their child's transition from home to school. The children progress through the program based on their development and mastered skills, not age. -Pueo (Phase 1): Children are introduced to a more structured learning environment. It focuses on colors, shapes and letter recognition. The class meets twice a week for 2 hours. It requires parents' 100% participation. -Honu (Phase 2): Children entering Honu have some basic, fundamental preschool abilities and are inspired to work independently, explore academics, use their creativity, and learn the importance of a daily routine. Honu focuses on phonics, basic math skills and writing. The class meets three times a week for 3 hours. The focus is on strengthening family sustainability through activities that encourages family participation. Hoa Aina O Makaha is a resource to the schools by providing interactive hands-on activities that broadens understanding of subject matter and enhances classroom lesson plans; activities include gardening projects and animal care. Small scale agricultural farming produces herbs and other produce.

Serves current and former foster youth in Hawai‘i who are transitioning out of the foster care system, into adulthood. Youth identified as requiring intensive mental health services may be enrolled with the Department of Health (DOH) Family Guidance Center (FGC) located in the school district of their home school. They are assigned an FGC Mental Health Care Coordinator (MHCC) at that time. The youth may, and often will, continue to receive School-Based Behavioral Health (SBBH) services and supports from the Department of Education (DOE) in conjunction with the intensive mental health services.

Transitional Family Care for youth 5 - 17 years of age with emotional and/or behavioral disturbances, Hale Kipa's Therapeutic Foster Care Program provides community-based services within a family home setting. The 3-9 month program is often a "step-down" from hospitalization or a residential setting and can be helpful in transitioning youth from a higher level of care to successful integration into the community.

Kahuku was the site of the first Rainbow School and has been operating continuously since 1972. We serve approximately 65 children ranging in age from 2 years to 5 years old. Our program is licensed by the State of Hawaii and all staff meet state licensing requirements. We accept DHS – Child Care Connection, Open Door, First-to-Work, Kamehameha Schools’ Keiki Pauahi Scholars Program, and Federal subsidies. All tuition payments are done through the FACTS Tuition Management program. Your child’s Form 14, current TB clearance and DHS 908 Form are required upon acceptance into the school. Parents are also responsible for a non-refundable registration fee of $175 upon acceptance. If there are no availabilities, your child’s application will be placed on the center’s waitlist. (Kumukahi Health + Wellness, Hawaii Island HIV/AIDS Foundation) Provides STD Testing and Treatment,HIV and Hepatitis C Testing, Sexual Health Counseling, PrEP Navigation Services, Client Services for people living with HIV and their families. Assists with daily living needs, financial aid for housing, food and medicine. Offers individual counseling, nursing, and social service case management. Accessing all aspect of medical care including vision, dental and drug assistance. Qualifying for public and private aid programs for housing and financial assistance, home health services, essential transportation, etc, Counseling and referral services including mental health, harm reduction and return to work programs, disability claims and a fully stocked food pantry on-site.

Early Childhood Services are provided to children three to six years old with delays in which age appropriate milestones are not yet met. These children typically fall outside the scope of the Department of Education & Special Education Services. The needs of each child are met through professional therapeutic services designed for the child to help them thrive in their home, preschool and community. Our goal is to better prepare the child for kindergarten and beyond by using family engagement techniques and strategies.
