(Transitional Family Homes) Provides a temporary out-of-home placement living environment for youths, ages 12 years old to 22 years old, that require additional resources to accommodate their emotional, behavioral, and mental health issues. Together, the staff and foster families provide youths with a system of support, counseling and skill building services. This service offers youth the opportunity to live with a trained transitional family who provides them with support and guidance. Services are offered to each youth based on their individual needs. Youths are provided with individual counseling and family counseling with the ultimate goal of reunification with their family.

The Respite Home program provides respite care as needed to families that are linked with the Department of Health Child and Adolescent Mental Health Division (CAMHD). The families that use the Respite Home therapeutic living program have behaviorally challenging youth in their homes and have used all of their resources to be able to maintain them on a constant basis. Respite Home services are designed to give these families short-term dependable relief to allow them to rejuvenate and provide a more stable family environment upon the youth’s return. This will hopefully decrease the potential for abuse and neglect and prevent placement of the youth in a more restrictive setting. (CFS Gerontology Program - Ohana Care) Caregivers who provide the primary care to individuals who are disabled, frail, and/or isolated residents on Oahu, who are 60 years of age or older, on an as needed basis are eligible for case management, counseling, and training services. Training topics may include: ADL/ADL limitations, caregiver’s role, disease prevention and remediation, access to available resources, stages of disease and options/ referrals in legal or financial related matters, RCI-REACH, and Arthritis Foundation related training. Staff will also provide information and referral sources when needed. Designed to improve the quality of life for families, care receivers, and caregivers by offering a continuum of quality care that is flexible and individualized in order to prolong the care giving relationship. The program actively assists the care receiver and the caregiver in implementing an individualized, strengths based service plan on how to reduce stress and link with the appropriate community resources. The program also provides supportive counseling.
