Mana`olana, Komohana, Lawehana and Kome Transitional housing sites provides safe and decent shelters with case management and assistance towards permanent housing by addressing obstacles which prevent homeless persons from obtaining and retaining permanent housing through a coordinated effort of health, housing, financial and social services.

Early Childhood Services are provided to children three to six years old with delays in which age appropriate milestones are not yet met. These children typically fall outside the scope of the Department of Education & Special Education Services. The needs of each child are met through professional therapeutic services designed for the child to help them thrive in their home, preschool and community. Our goal is to better prepare the child for kindergarten and beyond by using family engagement techniques and strategies.

Provides services to immigrants with low-income to enable them to stabilize their immigration status in the country. The program assists applicants for US Citizenship, relative petition change of status and other family immigration petitions with the US Citizenship and Immigration Services.

(West Hawaii Processing Center - Ka'u) The federal Supplemental Nutrition Assistance Program (SNAP) provides crucial food and nutritional support to qualifying low-income and needy households, and those making the transition from welfare to self-sufficiency. The SNAP processing center provides public assistance, food stamps (SNAP) and financial assistance, to eligible individuals and families. They handle new applications and ongoing assistance. A paper copy of the application can be downloaded, printed, signed and submitted via US postal mail or dropped off in the drop box. Offers weekly individual or group counseling focused on teaching independent living skills for youth aged 12 to 25. Maintains a training apartments from young men aged 17.5 to 19 who are in transition from foster care, group homes, shelters or institutions and also a home for young women transitioning out of foster care to independent living (training apartment participants are expected to have jobs and pay for rent and food). Also a home for pregnant and parenting teens.

(KTLC, Kumuhonua, Kumuhonua Transitional Living Center) Kumuhonua Emergency Shelter provides transitional housing and case management services for single adults, couples, and adult families (18 years and older) who are homeless or at risk of homelessness. Low-barrier to entry maintained.

(PATCH POD Program) The Preschool Open Doors (POD) program provides families with low and moderate incomes subsidies to send their children to preschool for up to two years prior to entering kindergarten.

Perinatal support services provides case management services to women who receive their prenatal care at Waianae Coast Comprehensive Health Center (WCCHC) from the time they enter prenatal care until 3 months postpartum. Perinatal support services also offers prenatal classes to pregnant women and their partners in the Waianae community.

Primary Care health & social services including diagnoses & treatment of acute and chronic diseases. Provides physical exams and immunizations. Offers specialty care in internal medicine, obstetrics, gynecology, pediatrics, psychiatric/behavioral health, optometry, and dentistry. Provides midwife services from prenatal to delivery and postpartum. Provides health, education, outreach and case management. Provides WIC (Women, Infant, & Children) nutrition services. Provides flu vaccines. Also provides a Smoke-Free Families Program.
