(Hui Malama Ola Na Oiwi) We provide disease prevention and health promotion services through advocacy, accessibility, and education. We offer Case Management Services (such as home visits to conduct blood pressure checks, glucose checks, assistance with applications for medical insurance), and referrals to other health-related agencies. We also provide Health Education (Diabetes, Hypertension, Nutrition, and Tobacco Cessation), Traditional Healing Workshops and Classes, as well as one-to-one services by referral. Specialty Transportation provided to medical-related appointments (based on needs and availability)- for individuals restricted to a wheelchair or gurney.

Promotes water enrichment and aquatic readiness activities for children and their parents. Skills are developed around five main components: personal safety, personal growth, stroke development, water sports/game, and rescue.

(Domestic Violence Advocacy) Families who are enrolled in Temporary Assistance for Needy Families (TANF) or Temporary Assistance to Other Needy Families (more commonly known as “welfare”), are required to be engaged in activities that will ultimately lead them to self-sufficiency. Program recipients for whom issues of safety and domestic violence are interfering with their progress can call upon Domestic Violence Advocacy to help them find support to change their situations. Services include: assessment, service planning, counseling services, case management and linkage and referral. Provides client advocacy for women referred by Department of Human Services (DHS) in need of case management and support to leave domestic violent relationships. Six month time period given to provide victims the opportunity to gain focus and perspective of their personal goals and career choice.

The federal Supplemental Nutrition Assistance Program (SNAP) provides crucial food and nutritional support to qualifying low-income and needy households, and those making the transition from welfare to self-sufficiency. The SNAP processing center provides public assistance, food stamps (SNAP) and financial assistance, to eligible individuals and families. They handle new applications and ongoing assistance. A paper copy of the application can be downloaded, printed, signed and submitted via US postal mail or dropped off in the drop box. (Mothers Milk) Hawaii Mothers' Milk, Inc. (HMMI) is a comprehensive breastfeeding resource center, for both families and health professionals in the State of Hawaii. HMMI provides services to new mothers statewide through our telephone warmline, individualized breastfeeding counseling, rentals and sales of hospital grade and/or home purchase breast pumps and supplies. Seeking volunteers to assist with clerical and other nonpatient duties. Donations accepted. This program offers lactation counseling and breastfeeding support, breast pump rentals, and breast pump sales.

(MDA Summer Camp) Each summer, kids and young adults with neuromuscular disease attend overnight camps around the United States at no cost to their families. Note: Currently there are no in-person camps in Hawaii. Please see the web site for virtual camp options. Campers may also apply to attend a camp in another location of the nation.

(PATCH POD Program) The Preschool Open Doors (POD) program provides families with low and moderate incomes subsidies to send their children to preschool for up to two years prior to entering kindergarten.

City and County of Honolulu youth can use the Youth HOLO card to pay for rides on TheBus, and in the future the HandiVan, and the rail system on June 30, 2023. Provides a comprehensive recreational program in the community. Facilities include a multi-purpose building. There are three program periods throughout the year: Fall, spring and summer. Activities are geared for all ages, including classes in arts and crafts, Hawaiiana, games and sports, music, seasonal activities and crafts, and excursions.
